Summer well checks and health forms

We wanted to pass along some helpful tips about summer well checks and school forms. As you probably know, summer is the prime time for annual check-ups. If you are planning to schedule your child’s well visit for the summer time, please contact our office as soon as possible to schedule your child’s or children’s appointment.

Also, many schools require medical forms be filled out in advance of the new school year. This means that our offices will receive lots of requests to fill these out. Here are some suggestions to make this process as smooth as possible:

  • Please allow two weeks for your doctor to complete your child’s medical form. If you need the form filled out sooner, there is an expedited surcharge.
  • Please pay the form fee at the time you drop it off at our office. This will allow us to return the forms to you as quickly as possible.
  • Please have all parent portions of your child’s health form filled out before your physician fills out the form. In addition to being helpful for your physician to review this information, many forms actually require us to review this information.
